The plan participant is responsible for utilizing the plan’s services Non-used services are non-refundable
This plan is non-refundable.
The plan renews automatically unless you opt out. If Plan expires, there is a $50 fee to reinstate the plan. For auto renewal, you payment info will be saved with our payment processor.
